Self-Certificate

If you have been ill for 7 days or less you do not require a Sick/Fit Note. You will be required to complete a Self Certificate form.

Please click here for the Self Certificate Form – you will be required to print it, fill it in and hand to your employer.

Sick/Fit Note

If you have been ill for more than 7 days, you would need to speak to one of our clinicians for Med3. Please contact the surgery on 0151 244 4550 at 8am Monday to Friday to book an appointment with one of our clinicians.

Additional Sick/Fit Note

If you already have a Sick/Fit Note for a recurring illness, you may not require an appointment with one of our clinicians. You can request this through PATCHS or contact the surgery on 0151 244 4550 after 10am and a member of our team can assist you with your request.
